Unwillingness to understand may have to do something with it (part of the us/them principle).
We have some areas here in Germany (esp. the South) where you can find people who simply refuse to understand High German. In France it is said that quite a lot of people will understand you until they notice that you are not from the area.

I don't have much trouble understanding most of the accents though I sometimes find some of the Yorkshire word usage obscure. The Reginald Hill mysteries with Andy Dalziel have helped though.

I've always been a big fan of British tv. I know lots of people who just won't watch anything with a British accent. I pity them. Life without Monty Python is too awful to contemplate.

Maybe Monthy Python is not the best example because they on occasion used nearly uncomprehensible accents deliberately. I have heard that in Britain some extreme examples are now sometimes subtitled.

Generally (i.e. not limited to Monty Python) I get in trouble with understanding when there are strong background noises or the soundtrack needs some remastering (independent of used accent).
But English is not my native tongue.

I love all Terry Pratchet especially discworld, my last 2 out of 3 drama productions have been from the discworld series, we've done "Guards! Guards!" and this year we did "Men at arms", they were awesome, I was the librarian and brother dunikun in "Guards! Guards!" that was SOOOOO AWSOME!!!! and I was main character in "Men at arms" (Carrot) That was fun...I have to say the politician has to be my favorite, oh him and Nobby...Good ole' Nobby...What a champ...

Actually, upon sober thought, Sean Connery would make an excellent Commander Vimes

Vimes could be an American (Carrot is the British Bobby, Vimes the American Fed).
Pratchett writes that he imagined Vimes as looking similar to Pete Postlethwaite (spelling?) though most people he has talked to thought of Clint Eastwood.
Christopher Lee could voice Death (as in the animated version). If he were a wee bit younger he could also play the Patrician (according to Night Watch he is only about 50, just a few years older than Vimes).
That disqualifies Peter O'Toole too.
Maggie Smith would be perfect as Granny Weatherwax, I think (unfortunately playing no part in GG).

The age of the actor doesn't matter for the Patrician, television can make him any age! But you could still portray him as a 70 year old anyway...
